A gravestone in the form of a terrazzo slab, reinforced, rectangular in shape, with an epitaph inscription executed on a marble slab.

Comments on the state of preservation and visible restoration:

The gravestone is practically completely destroyed, most of it covered with earth and overgrown with grass. A large part of the slab damaged and chipped (also the marble slab with the inscription) as a result of locating in close proximity to a new burial place in the plot that overlaps Bogusław Sługocki's plot.
