Burial site marked by obelisk on plinth and three-tier base. Obelisk on its own moulded plinth. On the front a bas-relief, glazed cross with a floral branch, below which a scroll-shaped plaque with an engraved inscription. Glazed base, third step with chamfered upper edges. On the pedestal, on the front, a glazed panel framed by fluting, in it a crossed, bas-relief cross and an anchor, above a bas-relief burning heart.

Inscription:

TU SPOCZYWAJA / ANDRZEY BEREZOWSKI / *1860 +25/VIII 1922 / I ŻONA JEGO / ANNA / * 1860 + 1/III 1933 / Oczekują zmartwychwstania //.
