Additional information:

According to Wacław Wejtko's records the plaque was located under the tombstone of Lenartowiczowa (probably http://cmentarznarossie.info/obiekty.php?id=10324), in 1934 it was moved to another place and in 1936 the plaque was kept by the Cemetery Board. Wacław Wejtko marks the fixing hole in the description. It cannot be ruled out that the plaque came from the catacombs. Aleksander Śnieżko gives the location of the plaque as Catacombs C, row III-25
