Burial place originally marked by cross on plinth and base. Cross not preserved. Base and plinth with chamfered top edges. An engraved inscription on the pedestal at the front.

D. O. M. / Tu spoczywają zwłoki / Anieli / z Hereskich / KOŁKOWSKIEJ / Ur. 4 Stycznia 1806 / + 4 Sierpnia 1879 r. //.
