Headstone marked by a stele supported on the left by a plinth, on a two-stage base. Stele with selected concave face. On the front an engraved inscription, above which a convex carved cross. On the second step of the base to the front in the upper right hand corner an engraved signature.

Ś. + P. / HELENA / z MICHALSKICH / PETRASZ / 2|VII 1906 + 11|I 1933 / PROSI O MODLITWĘ //.
