Burial place originally marked by cross or figure on plinth. The plinth slightly tapering towards the top with chamfered corners. Engraved inscription to the front. To the right an engraved frame with ears closed with a donkey's back arch at the top, gabled at the bottom. In it an engraved inscription.

Additional information:

Given the history of the cemetery, the form of the gravestone and the burials of the Pisz family, the first of the readings of 1823 is unacceptable. Probably the date in question is 1923, and the number 8 is the result of a stonemason's error.

Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i opublikowany w książce "Cmentarze dawnego powiatu tarnopolskiego", t. 1 i 2, pod red. A. S. Czyż i B. Gutowskiego, Warszawa 2017-2019 roku. Karty inwentaryzacyjne opublikowane zostały w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
