Burial site marked by an obelisk on a two-stage base and plinth. In the upper part of the obelisk a relief cross. The second step of the base with moulded upper edges. In the front face of the plinth an engraved inscription.

Ś. + P. / KAROL – MARYAN / NIEMENTOWSKI / NAUCZYCIEL SEMIN. NAUCZ. W TARNOPOLU. / * 1817 + 1894 / Żył dla drugich i uczył / życia dla drugich. //.

Inwentaryzacja cmentarzy dawnego powiatu buczackiego wykonana została w latach 2017-2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i opublikowany w książce "Cmentarze dawnego powiatu tarnopolskiego", t. 1 i 2, pod red. A. S. Czyż i B. Gutowskiego, Warszawa 2017-2019 roku. Karty inwentaryzacyjne opublikowane zostały w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
