A rectangular ground tomb, covered with a slab, at the head of which is a stele on a two-stepped pedestal and base. The stele tapers towards the top with a convex carved cross on the stele front. The second step of the pedestal with chamfered upper edges. An engraved inscription to the front. The whole is enclosed by a cast-iron fence, with a gate on the front on the axis. On the front wall of the tomb, to the right, an engraved signature.

Ś. + P. / TOMASZ / ŻUKOWSKI / ZMARŁ 5 LUTEGO 1937 ROKU / W WIEKU LAT 77. / WIECZNE ODPOCZNIENIE RACZ MU DAĆ PANIE. //.
