The burial site marked by an obelisk on a pedestal and three-stage base. On the obelisk at the front, in the upper part, a relief cross, below, convexly carved two olive branches.
On the pedestal to the front a panel with quarter circular chamfered corners, within which is an inscription in engraving. The bases are glazed.

Inscription:

TU / SPOCZYWA / ANTONINA PACHOŁEK / Z DZIEĆMI. EDIO LT. 2 I JAN / ŻYŁA LT. 26. UMARŁA 15/4 1925. / PROSI O MODLITWĘ //.
