The plaque commemorates the thirteen Polish Navy sailors who fell on 16 June 1942 participating in Operation Harpoon. The marble plaque is located on one of the columns in the upper gardens of Barrakka, Bastion of Saints Peter and Paul in Valletta. It was created on the initiative of the Wreck Expedition Association. The bilingual (English and Polish) plaque was placed on 16 June 2015, the anniversary of the sinking of the destroyer ORP Kujawiak. Under the motto 'So that we never forget them', the words of Gen. Stanislaw Maczek read: "Poles fought for the freedom of many nations, but they died only for Poland".
