The burial place is marked by a two-step tombstone, at the head of which is an obelisk on a three-step pedestal. The obelisk features a bas-relief cross on the front, with a laurel wreath tied with a ribbon below. First step of the pedestal with chamfered upper edges. Second step of the pedestal low, with profiled upper edges. The third step of the pedestal covered with a wider profiled panel, on the front with a medallion, decorated with stylised floral ornament. On the front, in a stepped panel, a plaque with an engraved inscription.

Inscription:

JULIAN MOTRYCZ / APTEKARZ i OBYWATEL / m. Monasterzysk / żołnierz z 1863 r / + 25 maja 1905 r. przeżywszy lat 61. / POKÓJ JEGO POPIOŁOM //.
