Headstone marked by a stele on a pedestal in a concrete mantle with a band in the form of a coffin lid. The stele is bevelled at the top, with a polished front, where the engraved inscription No. 1. On the band in front the engraved inscription No. 2. The whole is framed originally by ten cylindrical pedestals connected by doubled iron bars. The tombstone in a common section with tombstones Nos. 43, 44, 46, 47, 48.

1. Adolf / PTAK / zm. 26 Lut. 1892 r. / lat 55. / Tę pamiątkę poświęcają / żona i dzieci. / ANIELA / z NOWOKUNSKICH / PTAK / ZM. 27 IX 1920 R. LAT 70. //.
