Burial site originally marked by a cross on a plinth, on a two-stage plinth and base. Cross not preserved. Base with chamfered upper edges. First step of plinth with moulded upper edges. The second step of the pedestal high, tapering upwards, with profiled upper edges. Engraved inscription to the front. The gravestone enclosed by a fence consisting of a band and iron bars joined by slats. On the front left side a gate, inside the fence a bench. The fence is shared with gravestones Nos. 5, 6.

S. P. / Władzjmiesz / SŁAWIŃSKI / + 17 Octobra / 1896 r. / Pomnąc na chwilę / śmierci Na własne / skonanie / Westchnijcie zan do Boga O wierni chrześcijanie. //.
