The tomb is rectangular in plan, covered with a slab, at the head of which is an obelisk on a pedestal and base. A bas-relief inscription on the front of the obelisk. The base with chamfered upper edges. The plinth with moulded upper edges, on the front a bas-relief olive lamp, below a bas-relief inscription. On the panel four plinths with cast iron bars between them. On the front on the axis a secondary created entrance, accessed by eight steps.

Gravestone cleaned, most probably repaired, with a secondary engraved inscription at this and the neighbouring quarter: 2011./Cz. Polonsk //.
