Headstone marked with cross on two-stage plinth and two-stage base in monumentalised band. Latin cross on two-stage plinth. Second step of base with chamfered upper edges. On the first and second steps of the pedestal inscriptions engraved on three sides.The whole enclosed by a fence consisting of a concrete band and 11 cylindrical pedestals connected by a cast iron chain. An engraved inscription on the left side of the band: Cz.Polonsk 2011 (see notes).
