A tomb of rectangular plan, covered by a slab with two vertical elements - a cross stylised as a tree trunk on a rock-shaped plinth and two-stepped base, and a cast-iron cross on an irregular stele and base. On the plinth to the right a plaque in the form of an unfolded scroll with an oval photograph on porcelain and a relief inscription (1). A cast-iron cross with arms terminating in a stylised trillium form, on a plinth. The face of the cross excavated. On the stele at the front an engraved inscription (2). On the axis of the tomb to the front a two-leaf cast-iron door with entablature, decorated with fittings and two crosses. On the tomb slab a simple cast iron fence with a gate at the front.

2. S. P. OLES MAŁACHOWSKI / zm. 23 Marca 1892 r. / mając 2 lata i 8 mies. / O synu, synu nasz jedyny / Nas opuściłeś bez żadnej przyczyny / Bóg cię wzorze cnoty / Powołał do siebie / Bądź wola Jego święta / On nas złączy w Niebie / Stroskani rodzice tę pamiątkę / poświęcają. //.
