Burial place marked by cross on two-stepped plinth and two-stepped base. Cross on moulded plinth. Second step of base with chamfered upper edges. First step of plinth with chamfered upper edges. Engraved inscription to front (2). Second step of the pedestal with chamfered upper edges. To the front (1) and to the right (3), engraved inscriptions.

Inscription:

1) 1900 r. 15 Stycznia / stanął przed Bogiem / EDWARD / WASILEWSKI / Bądź Wola Twoja. //.

2) Ś. P. / MARJA z SIELANKÓW / WASILEWSKA / ZM. 7 KWIETNIA 1904 R. //. 3) S. + P. / ALINA / z WASILEWSKICH / KOZŁOWSKA / ZM. DN. 30. VI. 1919 R. / W KIJOWIE. //.
