Burial place marked by a cross on a two-stage plinth and base. Cross with chamfered edges, on moulded plinth. Base with chamfered top edges. First step of plinth with moulded upper edges. In front, a panel with quarter-circle chamfered lower corners, closed with a segmented arch, in which a bas-relief inscription is carved (1). To the right, a panel with quarter-circular chamfered corners with a relief inscription (2). The second step of the plinth tapering upwards, with profiled lower and chamfered upper edges. In front a panel with quarter-circular chamfered corners with a relief inscription (3). The whole, together with tombstones Nos. 75, 76, 77, 78 and 79, enclosed by a fence consisting of cylindrical pedestals connected by a cast-iron bar.

In Wacław Wejtka's card file, under the letter Po, card number 42, dated 1936. According to the file, the location of the gravestone is sector "D1, 2nd row, N53, from the Czapski tomb, to the end, by the road on the left". Next to the description of the gravestone the annotation: 'chain-link fence, together with N43, 45, 47, 48, 51'.
