The burial site marked by a cross on its own plinth, three-stepped plinth and base. The whole founded on a common band.Base in the form of rocks. First step of plinth with chamfered top edges. Second step of pedestal with moulded top edges, inscription engraved on front (2). The third step of the pedestal with a panel with a relief ending in a semicircle. In the panel, an engraved inscription (1). The plinth with chamfered upper corners, passing into a cross.

Inscription:

1. Tu odpoczywa s.p. / FRANCISZEK / CHLEBIK / zmarł 2 lutego 1909 / w 28 roku życia swego //.

2. Odpoczywaj w pokoju / drogi małżonku i ojcze //.
