The parish church in Brzeżany, originally as the Church of the Nativity of the Blessed Virgin Mary, is one of the most valuable monuments of sacred architecture in the eastern areas of the former Republic of Poland. The temple was founded by Adam Hieronim Sieniawski, Crown Chamberlain, and its construction was completed in the 1730s. According to other sources, the foundation initiative belonged to the entire Sieniawski family, the town's owners.

On 31 August 1625, the church was consecrated by the Lviv Archbishop Jan Andrzej Próchnicki under the name of the Nativity of the Blessed Virgin Mary and the Crucified Jesus. In the second half of the 17th century, extensive renovation and reconstruction in the late Gothic style took place.

In the 18th century, under parish priest Kryspin Cieszkowski, the church was renovated and consecrated again on 17 September 1775. Kryspin Cieszkowski enriched the church with numerous chapels with altars dedicated to St Anne, St Francis Xavier, St Joseph and St John of Nepomuk, among others. The church survived numerous destructions, including a fire in 1811, as well as devastation during the First World War. The financial support of Izabela Lubomirska and the involvement of the local community made it possible to rebuild it.

After the Second World War, the church was used as a gymnasium, which contributed to its deterioration. It was not until the 21st century, thanks to the commitment of Poles and lovers of the Borderlands, that a thorough renovation was carried out, including drying the foundations, replacing the roof and restoring the Gothic altar.

The church in Brzeżany is a unique example of borderland renaissance with distinct gothic elements. The building had a defensive character, which is emphasised by the preserved fragments of the fence and the bell tower from 1630. The austere form of the temple and the decoration of the interior reflect the spirit of the period, combining functionality with artistic beauty.

At the beginning of the 20th century, the pulpit was decorated with figures made by a representative of the Lviv school of rococo sculpture. Renovations in the 21st century restored many elements of the decor, including a beautiful Gothic altar funded by Polish donors.

In the past, the church had numerous chapel altars, including those dedicated to St John Cantius, St Francis Xavier, St Anne and the Virgin Mary of Sorrows. These elements highlighted the diversity and richness of religious worship in the shrine.

In 2018, the Polonica Institute carried out the conservation of the portal leading to the sacristy.
