Tombstone originally marked by a cast iron cross on a rock-hewn plinth. Cross perpetuated, lying beside headstone. Cross with arms terminating in a trilobed shape, on a plinth. At the intersection of the arms the mount is visible after the figure of Christ. In front a plaque with an engraved inscription. The whole is enclosed by a fence consisting of cylindrical pedestals connected by a cast iron bar.

Tu spoczywa / Syn nasz ukochany / Jaś / CZYŻEWICZ / zm. 1893 r. zył lat 2 / i 4 m. //
