Burial place originally marked by a statue on a two-stepped plinth and three-stepped base. The statue is not preserved. First and second steps of plinth with moulded upper edges. Third tier of plinth with simplified beam consisting of architrave and cornice, covered by hipped roof with triangular peaks on four sides and acroterions on corners. On the front an engraved frame enclosed by an overhanging full arch. In it an engraved inscription.

Inscription:

AGATA / MIŚKÓW […] / urodz. 188[7] / umarł[a] […] / roku //.
