An earth tomb on a rectangular plan covered with a slab, at the head of which is a cross stylised as a tree trunk on a rock-shaped plinth and a two-stepped base. The two steps of the base are glazed. On the plinth to the front a plaque with an irregular top edge where a damaged oval photograph on porcelain. Through the upper right-hand corner of the plaque a bas-relief floral wreath, the lower left-hand corner decorated with palm leaves. On it an engraved inscription. Burial crypt built of brick, covered with a barrel vault. Entrance from the front. There is at least one tin coffin inside.

ADAŚ / ukochany synek / BRONSŁAWA / i MARYI / CZAYKOWSKICH / * 13.V.1898 + 23.XI.1903. / „Puśćcie do mnie małe dzieci / Albowiem Ich jest królestwo / niebieskie„. //.
