Burial site marked by cross on plinth and two-stage base. Cross with capped arms on a high two-stage plinth. Through the intersection of the arms crossed by a relief ribbon. The face of the cross is cut out. The corners of the plinth framed by canellated pilasters with bases and heads with Ionic kimation. At the back a bas-relief chalice with host, cross and palm leaf, set on a book. To the front (1) and left (2) an engraved inscription.

Inscription:

1. GRÓB / MICHAŁA KOLAN / KOWSKIEGO / Plebana obr. gr. / kath. w PotoKu / zmarłego dn. 27 / Paźdź. 1847 w 65 / roku życia swojego //.
2. ГРОБ / MИXAИЛА Ko= / лянковcкиегo / Пapoxa гр. кaт. / з Пoтoкa o[…] / […] 27 Oкт / 1847 […] / […] //.
