Burial place marked by cross on plinth and base, in band. Cross on plinth. At the intersection of the arms a medallion with the relief inscription INRI. The plinth with a rectangular uplift. On the front a profiled panel closed with an overhanging segmental arch, in which is an engraved inscription.

S. † P. / HRUSZCZAK / MIKOŁAJ / HALENA / I ADOLFINA / O KTURYCH / NIE ZAPOMINA / W KANADZIE SYN ANTONIN //.
