ukr. Załiszczyky (Заліщики); inna nazwa: Zaleszczyki Małe
Burial site marked by obelisk on plinth and base. Obelisk surmounted by a simple cross. Base with moulded top edges, with lesbian kimation. Plinth covered by wider rectangular panel with Ionic kimation, acroterions on corners and pediments between. To the front a panel with a rhytic inscription.
Inscription:

Tu spoczywa Ś:P: / Magdalena z Wadtronom / Brylinska! / przeżyła lat 59 zmarła / 3go Stycznia 1863 / Spokój zwłokom jej //.
