ukr. Załiszczyky (Заліщики); inna nazwa: Zaleszczyki Małe

Burial site marked by cross on two-stage plinth and base. Cross on plinth. On the base at the front in the upper right corner an engraved signature. First step of plinth with moulded upper edges. The second step of the plinth with a low rectangular uplift framed by two flattened pyramids. On the front an engraved inscription.
