Burial site marked by a cross stylised as a tree trunk, on a rock-shaped plinth with rows of wooden beams. Cross with relief carved figure of Christ above which is a grapevine. At the front on the pedestal is the figure of an angel leaning against a beam with his right hand and holding a scroll with an engraved inscription in the other.

Inscription:

TU / Spoczywa / Maks RAWLUK / * 9 X 1874 + 22 X 1935 / Requiem aetaernam dona ei Domine. //.
