Burial place originally marked by a cross on a three-tier plinth and base. Cross not preserved, on moulded plinth. First step of plinth with chamfered upper edges. The second step of the plinth with a moulded cornice, interrupted in the middle, where a plaque with a ritual inscription is placed (4). The third step of the pedestal tapering upwards, enclosed with a stepped top, on the front (2 and 3) and on the right side (1) an engraved inscription. The whole enclosed by a fence consisting of cylindrical plinths connected by a cast iron chain. The fence is shared with gravestone no. 54.

3. S. + P. / OKTAWIA KOZAKOWSKA / ZM. DN. 15. VIII. 1919 R. / W WIEKU LAT 80. / POKÓJ JEJ DUSZY. //.
4. Stroskany mąż pomny miłości / szczęścia i spokoju jakiemi ota - / czała go w trzydziestu letniem / pożyciu pomnik ten dla Niej / poświęca. //.

Time of creation determined hypothetically based on form and dates of life. Beside the gravestone an overturned cross, perhaps it was the original finial of the monument.

State of preservation:

corrosion, mosses, lichens, cracks, non-maintained finial
