Burial place originally marked by a cross on a plinth banded in the form of a coffin lid. Cross not preserved, on a profiled plinth. Plinth with 1/3 height profiled sides. On the front a plaque with an engraved inscription.

Inscription:

Ś + P / WŁADYSŁAW / WOŁOSEWICZ / zm. 2 VIII 1917 r. w. l. 17 / POKÓJ JEGO DUSZY //.
