Burial site marked by cross on plinth, on concrete plinth. Plinth on base. Concrete band in front of plinth in shape of coffin lid. Wrought iron, openwork cross. Cross arms terminating in arrowheads. Cross in geometric patterns with escutcheons. Rosettes at the intersection of the cross arms. Gabled plinth. On the plinth a gabled plaque with an engraved inscription.

Inscription:

Ś. + P. / DOMINIK / SZYSZ / UR. 4 SIER. 1890 R. / ZM. 13 LIPCA 1925 R. / Boże ! / zbaw duszę Jego. //.
