ukr. Zbaraż (Збараж)
Burial site originally marked by a cross on a plinth and three-tier base. Cross not preserved. Plinth with moulded upper edges. On each side panels with chamfered quarter-circle corners, engraved inscriptions in three: front (1), right (2), left (3).

2. STROSKANE / DZIECI Z FAMIELII / PROSZĄ O TROIE / POZDROWIENIA / ANIELSKIE //.
3. UMARŁ DNIA 5 / LIPCA ROKU 1857 / ŻONA UMARŁA / DNIA 8 SIERP / NIA ROKU 1858 //.
