A gravestone marked by a three-tier headstone, at the head of which is a cross on a two-tier plinth and two-tier base. Cross on a plinth. At the intersection of the arms of the cross a relief figure of Christ, above a titulus. The first step of the plinth with profiled upper edges. An engraved inscription to the front. The second step of the plinth with a simplified beam consisting of architrave and cornice, covered by a wider panel, acroterions at the corners and a gabled gable. To the front, a relief carved figure of St Simon the Apostle in a panel with quarter-circular chamfered corners. The saint holds his right hand over his heart, a saw in his left. Dressed in a long ruffled robe and cloak. Long hair and beard. Shoes visible. Analogous frames at the back and sides.

Inscription:

TU SPOCZYWAIĄ / SZYMON. / CRANATER. / + ŻL [72] / ANNA + ŻL [...] / [IGNAC] + ŻL 12 / PROZZĄ ANIOŁ PAŃSKI //.
