The tomb is covered by a slab with contemporary fencing, at the head a statue of St Charles Borromeo on a two-stepped plinth and base. St Charles dressed in a long ruffled robe, shoes visible. He holds a cross in his right hand and a book in his left. He is wearing his hair short. First step of plinth with chamfered top edges. The second step of the plinth covered by a wider panel, below an oval panel originally with a porcelain photograph, on the front an engraved inscription.

Inscription:

Tu spoczywa / najdroższy syn / KAROL / SWIENTANOWSKI / żył lat 26 zamordo / wany p[rzez] [...] / [...] dnia 2.7.1941 //.
