The gravestone marked by a cross on its own moulded plinth, a two-stage base and plinth with a tombstone added to the front. The cross profiled on the front, at the intersection of the arms of the cross a relief medallion, within which is a relief carved head of Christ with long hair, shown in left profile wearing a crown of thorns. The pedestal surmounted by moulding, on the front an engraved inscription.

Inscription:

Tu spoczywają / [...] JAN PACISZAK / 1[8]06 ✝. 1876 / i żona / JOANNA 1805 ✝. 1877 / Proszą o Anioł Pański. //.
