Rectangular tomb with rusticated walls. Covered by a flat slab with a relief cross. At the front, on the axis, the entrance covered by an oblique panel with two cast-iron entablatures flanking the cross. On the sides of the entrance two rectangular panels. On the right a red sandstone plaque with an engraved inscription.

Inscription:

Ś. + P. / MARIA Z SZANDAŁÓW / KOCOWICZOWA / + 27/8. 1935. przeż. lat 72. / Prosi o Modlitwę ! //.
