Rectangular tomb, covered with a slab, at the head of which is a wall with architectural decoration. The wall is risalitic, built on a segmental arch with a concave-convex cornice. In front a panel with quarter-circular chamfered lower corners, in it a shallower irregular panel surmounted by a cross on which is an engraved inscription. The panel framed by relief ornamentation: palmette, acanthus leaves and rocaille. On the sides of the wall slivers passing into volutes surmounted by urns. Urns on moulded, corrugated feet with convex bellies. On the slab covering the tomb a band with rounded corners. On the front and sides, pedestals left over from a non-western fence. At the front of the tomb, on the axis, a bricked-up entrance opening framed by five steps, on the left a bench.

According to the file of Lucjan Uziębła, the gravestone framed by "two soldered amphorae with flaming torches - a fence of iron bars on decorative concrete posts".

Comments on the state of preservation and visible restoration:

The conservation of the tombstone was carried out in 2016 with funds provided by private donors Henryka and Czesław Okińczyc.
