The burial site was originally marked by a vertical element on a three-stage plinth and a two-stage base. The whole enclosed by a cast-iron fence consisting of cast-iron rods with decoration consisting of channelled mouldings. On the first and second steps of the pedestal, intaglio carved inscription plaques with an inscription in relief. A cast-iron bench set to the right of the gravestone, within the enclosure.

Ś.+ P./ Adolf/ Korsak/ ur. 24 Maja 1837 r./ zm. 18 Sierpnia 1903 r. //. 2: Pokój Twej duszy/ najdroższy mężu. //.
