The burial place marked by a cross on a plinth, placed on a two-stage base, on the plinth, on a matt grey plaque carved in stone, the inscription convex black smooth polished.

Inscription:

Tu spoczywa w Bogu / S.P. Roman Okulski, / żył lat 52, zmarł dnia 28 maja 1926 roku. / Wdzięczna pamięć o Tobie żyć / zawsze będzie w sercu kochającej żony. / Proszę o Ojczenasz. / Anna Okulska / ur. Babjanska / 1875.-1937.

Inwentaryzacja cmentarzy dawnego powiatu buczackiego wykonana została w latach 2021-2022 9 przez zespół specjalistów z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie i Muzeum Narodowego w Warszawie w składzie dr Paweł Drabarczyk vel Grabarczyk, dr. Bartłomiej Gutowski (kierownik projektu), dr Magdalena Olszewska dr Magdalena Tarnowska. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i opublikowany w katalogu Instyttutu Polonika. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
