Plaque with a five-sided panel at the top of the front with traces of a nonpreserved coat of arms, an engraved inscription below and a bas-relief skull with tibias.

According to Lucjan Uziębła's file, the plaque was located in catacombs A, front. The researcher also noted the existence of a coat of arms. In Wacław Wejtka's card file, under the letter Bo, card number 331, dated 1933. According to the card file, the old location of the tombstone was "catacombs A., IV. N13". Catacombs A, row IV-13
