Burial place marked by cross on plinth and base. Plinth with chamfered upper edges. On the front a plaque with an engraved inscription (1). On the right (2) and left (3) sides of the plinth an engraved inscription.

According to Lucjan Uziębła's file, the gravestone was originally enclosed by a metal fence. According to Wacław Wejtka's file, the inscription probably from an earlier gravestone: S + P. / z Jeleńskich / Emma / Dmochowska / Died 24 January 1919 / at the age of 55. //. In the file of Wacław Wejtka, under the letter J, card no. 186. According to the file, the old location of the gravestone is the sector "ZW Cemetery. K12 (plan N 27)".
