Burial site marked by an obelisk on a two-stepped plinth and base. The obelisk closed with a semicircle and framed by sections of a circle. On the front a medallion with a bas-relief of the head of Christ wearing a crown of thorns, below a drapery hung with two nails, below an ear plaque decorated at the bottom with an oak branch. On the plaque an engraved inscription.

First step of plinth with chamfered top edges. The second step of the plinth with moulded top edges, where the engraved signature is to the right.

Inscription:

 Ś+P / KALUS CZOP / * 6/8 1906. / + 11/10 1920 / z Semenowy //.
