Burial place marked by a cross on a base and two-stage plinth. Cross with capped arms, on plinth, flanked by figures of kneeling angels. On the front a relief carved figure of Christ, above a titulus, below a skull. The first step of the plinth with moulded upper edges. On the front an engraved inscription. The second tier of the pedestal with a simplified transom consisting of a knuckle frieze and cornice, covered by a hipped roof. On the front a panel closed with a full arch, inside a relief figure of Our Lady of Sorrows, portrayed in full figure, in counterpoint, frontally. She is wearing a dress, cloak and headscarf.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
