Headstone marked by a statue of St Stephen on a two-stage plinth and three-stage base. The figure on its own base. The saint holds a palm branch in his left hand and a book of stones in his right. He is dressed in an alb and dalmatica. Long hair. Shoes visible. On the front of the first step of the pedestal an inscription engraved 1. The second step of the pedestal with a simplified beam consisting of an architrave and cornice, covered by a wider panel. On the front a panel enclosed by a full arch with a bas-relief figure of St Francis. With her hands she supports three loaves of bread. She is dressed in a ruffled robe and mantle, with a veil on her head. Below the engraved inscription 2.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
