Burial place originally marked by a statue on a two-stage plinth and a two-stage base. On the first step of the plinth, an engraved inscription (2) on the front wall. Second step of plinth enclosed by simplified beam with arcaded frieze. On the side walls empty deep niches closed with semicircular arch. Under the niches there are engraved inscriptions. On the right wall, inscription - Agnes. On the left wall an inscription - Thomas. On the front wall an engraved frame and an engraved inscription (1).

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
