Headstone marked by a stele with concrete band. The stele closed with a semi-circular relief, with an engraved inscription to the front. On the band two pedestals connected to the stele by cast-iron bars, on the front of the band in the upper left corner an engraved signature.

Inscription:

Ś. + P. / ZYGMUNT KONDRACKI / UR. 14 - X - 1868 R. ZM. 29 - XII - 1931 R. / Cześć Twej przezacnej duszy / Umiłowany i najlepszy mężu. / Żyłeś sercem i w sercach naszych / żyć będziesz na wieki. / BOŻE ZBAW GO. //.
