Burial site originally marked by a cross on a two-stage plinth and base. Cross perpetuated, lies beside gravestone. Cross with rhomboidal terminated arms, chamfered, on plinth. At the intersection of the arms an engraved medallion with the inscription IHS, below a smaller cross. The first step of the plinth with chamfered upper edges. On the front an engraved inscription (1). The second step of the plinth with chamfered upper edges. On the front a stepped panel with engraved inscription (2).

2. TU SPOCZYWA / JAN / PAWLUK / SYN ANTONIEGO / * 1885. + 16/6 1923 //.
