The burial site was originally marked by a cross on a two-stepped plinth and two-stepped base.
Cross perpetuated, not preserved.
Fragment of lower arm of cross on its own moulded plinth. First step of plinth with moulded upper edges. Engraved inscription to front.
The second step of the pedestal with chamfered upper edges, on the front a panel enclosed in solid slate with a convex carved figure of Saint Anthony. The saint is depicted wearing a long robe and veil. She holds a chalice in her right hand and a palm branch in her left.

Inwentaryzacja cmentarzy dawnego powiatu buczackiego wykonana została w latach 2021-2022 9 przez zespół specjalistów z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie i Muzeum Narodowego w Warszawie w składzie dr Paweł Drabarczyk vel Grabarczyk, dr. Bartłomiej Gutowski (kierownik projektu), dr Magdalena Olszewska dr Magdalena Tarnowska. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i opublikowany w katalogu Instyttutu Polonika. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
