Burial place marked by a statue of St Anne teaching Mary on a two-stage plinth and two-stage base with an attached gravestone. The figure on its own plinth with chamfered side edges. Saint Anne depicted in counterpoint, wearing a long ruffled gown with her shod feet protruding from underneath. A cloak and short veil are visible. With her right hand she holds up a book and the hem of her cloak. With her left hand she touches Mary. Mary facing her mother, wearing a long gown with a belt at her waist, the two ends of which fall over the front of the gown. Visible are the shod left foot and long wavy hair. Hands folded in prayer. First step of plinth with chamfered top edges. Second step of plinth slightly tapering upwards, terminating in a simplified beam and covered by a wider hipped panel. An engraved inscription to the front.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
