Burial site originally marked by a statue of St Anthony on a two-stage plinth and three-stage base. Destruct. St Anthony on his own base, depicted frontally, in Franciscan habit, holding the infant Jesus on his left hand, in his right hand holding a palm branch. The first step of the plinth with moulded upper edges. The second step of the pedestal with an engraved frame on the front with quarter-circle chamfered corners, in which an engraved inscription.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
